Do Bras Prevent Sagging?

What actually drives sagging
Ptosis, the clinical word for it, is tissue and skin behavior over time, and its known drivers are stubbornly unglamorous: genetics first, then pregnancy, larger swings in body weight, smoking (which degrades skin elasticity), higher breast volume, and years. Notice what is absent from the list: bra-wearing hours. The supporting structures involved are ligaments and skin, not muscle; they neither strengthen from support nor atrophy from its absence in any demonstrated way, which dissolves both halves of the folklore at once.
The counter-claim, that bras cause sagging by making tissue "lazy," rests on one small observational study of young athletes whose own author cautioned it could not be generalized to the wider population. Neither direction of the strong claim survives contact with the actual evidence, which is thinner than either camp admits, and honest fitting advice lives comfortably inside that uncertainty: wear what feels good, for reasons that are real today.

What a bra actually buys you
Everything a bra does, it does while worn, and that is plenty. It shapes your silhouette today. It redistributes weight for comfort across long hours, the mechanics of the back-pain question. And during exercise it dramatically reduces breast movement, which is the difference between a comfortable run and a painful one at nearly any size: the case for a real sports bra is comfort and pain, on solid ground, rather than a promised future shape.

The one arena where support has an unambiguous, felt job is movement.
